Dr Chikul Mittal is a seasoned clinician administrator with over 16 years of experience in various re-structured hospitals within Singapore.

A public health specialist trained in community geriatrics, he focuses his clinical skills in care transitions from hospital to home, and caring for vulnerable elderly in the community. As a strong supporter of ‘out of hospital’ care he is actively involved in advocacy around access for mental health, community-based care and adult vaccinations.

As an administrator, he has wealth of experience in various aspects of healthcare management including but not limited to clinical quality, accreditation, governance, analytics, care integration, innovation and development of new models of care. With keen interest in translational research, he has published several articles on care-delivery re-design.

In his current portfolio as Medical Director, Special Projects, he oversees model of care, medical standards and governance for 2 Transitional Care Facilities (TCF@West and TCF@East).

Dr. Chai graduated with Bachelor of Medicine, Bachelor of Surgery (Honours) from the University of Nottingham (United Kingdom). She subsequently obtained her Graduate Diploma in Family Medicine from the National University of Singapore.

Her previous rotations in internal medicine, emergency medicine, geriatric medicine, community hospital care, and pediatric medicine have provided her with a broad clinical foundation, allowing her to deliver holistic, patient-centered care across complex medical and psychosocial needs.

Dr. Ang is a board-accredited Family Physician with a Master’s degree in Family Medicine and over 12 years of clinical experience caring for patients across hospitals and polyclinics in Singapore.

Passionate about community-based medicine, Dr. Ang has a special interest in geriatric and general palliative care and finds fulfilment in supporting patients and their families through life-limiting illness with empathy and dignity.

Teaching is an important part of Dr. Ang’s professional practice. As a teaching faculty member in family medicine education in Singapore, he is actively involved in mentoring and training junior doctors, dedicated to guiding and empowering the future generation of family physicians.

In his current role as Medical Lead, Dr. Ang works closely with multidisciplinary colleagues to deliver holistic, team-based care, and oversees the nursing team in the day-to-day operations of the Transitional Care Facility @ West.

Dr Joey Seah has been practising as a clinician since 2017, with a strong interest in community-based elderly care. He has received broad clinical training across multiple specialties, including Geriatric Medicine, General Medicine, Cardiology, Endocrinology, and Renal Medicine.

He currently serves as the Medical Lead at Transitional Care Facility @ East, where he provides clinical leadership and oversees multidisciplinary teams comprising doctors, nurses, therapists, and medical social workers. In this role, he supports coordinated, patient-centred care for individuals transitioning from acute hospital settings to the community.
